Riddle Me This, Batman:

Holy Ho-Ho's and Honeybuns Batman, I need help!
So I am writing a paper now for ASSESSMENTS class.
It is a class on assessing people in preparation for counseling.
I believe that poor nutrition contributes significantly to disabling conditions and depression, so I am writing about nutritional assessment.
The instructor, who is one of the strictest and most thorough and caring/fair professors, has given up the battle on research restrictions and encouraged me to "google" nutritional assessments.
I however, feel it is worthy of actual empirical evidence in research, and so am going to the library now to look into it.
Basically, I must find and define two examples of how to efficiently assess a person's nutritional intake and overall eating habits.
I would like to combine exercise too, but not sure if that will be too broad.
The assignment just calls for a focused assessment tool (a report on a tool) or design model that can be incorporated into an initial intake, follow up and closure assessment.
SO,
Anybody have any suggestions on assessment of nutrition? It should be a model that is already established.

true confession
i did not study for my assessments exam that will take place in.....2 hours.
i co-wrote a 40 page study guide, consolidated it down to 25 pages and facilitated THREE study groups, but me personally? never sat down and read or studied the thing, or the material, or the lecture notes.....yeah, and now i am blogging.
